To verify your eligibility as a Conrad's customer, it is important you complete the following form with as much detail as possible.
* Every 90 days receive a $10 reward certificate if your accumulated Conrad's Tires, Maintenance or Repairs purchases are $200 - $499 OR a $25 reward certificate if your accumulated Conrad's purchases are $500 or more.

The Invoice Number is located in the upper right-hand corner of your invoice and always begins with a case-sensitive letter or special character.
Please enter the invoice number exactly as it appears on the invoice.
